2. Complete Paying - in - slips in quadruplicate.
3. Please quote your File Number and Year in all Paying - in - slips.
4. Forward the remittance and the paying – in – slips to any branch of the Bank of Ceylon of Galle/ Mathara/ Hambanthota district.
5. If payment is made in cash Complete Paying - in - slips in quadruplicate and present to the any branch of the Bank of Ceylon of Galle/ Mathara/ Hambanthota district
6. Copy of Paying – in – slip and Return (දපස/ආකො/ඖෂධ 03) send to the Galle/ Mathara/ Hambanthota office
